PROXY ACCESS
A parent may be able to add a child (age under 16) to the parent's account as a proxy user.
Please download this Consent Form:
Alexandra Surgery Proxy Online information and application form
After you have filled and signed the Consent Form, please send it together with copies of the parent's ID and child's ID, to the practice.
You may do so by email or by post.
We can then add the parent to the child's account.
